Healthwatch survey – Joined up care

Health and care services are working to become more integrated in the care they provide for patients. This means care from health professionals and across different services will be more ‘joined up’ around people’s needs.

Healthwatch Oxfordshire would like to hear from people (based in Oxfordshire) about their experience of joined up care, through a short, anonymous online survey.

If you wish to take part, please share your thoughts through this survey, which is open until the end of August 2023. 

You do not have to say who you are and Healthwatch will not identify you. 

Their survey findings will help to shape the support people need, and improve services.
